Notice 2004-006 - Revised CNQ PIF (Form 3)
REVISED CNQ PERSONAL INFORMATION FORM (FORM 3)
In accordance with federal privacy legislation, CNQ�s Personal Information Form has been revised. The only changes are to include CNQ�s personal information collection policy and to obtain an acknowledgement from the person completing the form that he or she has read and understood the policy.
The revised FORM 3 is effective immediately. Personal Information Forms already filed with CNQ do not have to be resubmitted. PDF copies of the new form are available on the CNQ website at www.cnq.ca.
